In recent decades there have been few advancements in disease management. Advanced heart failure treatment (AHFT) centers provide state of the art care for patients with HF including use of left-ventricular assist devices (LVAD), heart transplants, and integrated palliative care. Rural dwelling HF patients have worse health outcomes and have difficulty accessing AHFT centers which tend to be located in urban areas. Shared care treatment models have been shown to improve access to specialty care, but have not been widely studied in advanced HF patients. This quality improvement project assessed the experience of patients participating in a new AHFT shared care program. 000010046 542__ $$fIn copyright - single owner 000010046 650__ $$aRural Health Services$$031299 000010046 650__ $$aQuality Improvement$$039456 000010046 650__ $$aDisease Management$$031650 000010046 650__ $$aHeart Failure$$019939 000010046 650__ $$aHealth Disparate, Minority and Vulnerable Populations$$013779 000010046 6531_ $$anursing shared governance 000010046 691__ $$aSchool of Nursing$$041370 000010046 7001_ $$aAllen, Tiffany L.$$uOregon Health and Science University$$041354 000010046 7201_ $$aSoffer, Jonathan$$uOregon Health and Science University$$041354$$7Personal$$eChair 000010046 8564_ $$9607c8f27-4ef4-4067-a815-4d82c1519371$$s1487836$$uhttps://digitalcollections.ohsu.edu/record/10046/files/Allen.Tiffany.2023.pdf 000010046 905__ $$a/rest/prod/jw/82/7c/34/jw827c34r 000010046 909CO $$ooai:digitalcollections.ohsu.edu:10046$$pstudent-work 000010046 980__ $$aTheses and Dissertations
