The veteran suicide rate is 1.5 times greater than the civilian population, and lack of access to specialty mental health (SMH) care has been identified as a contributing factor. One way to improve access to SMH is by transferring psychiatrically stable patients from SMH to primary care (PC) for further management which will increase availability in SMH provider schedules for patients with acute mental health needs. The purpose of this project was to increase SMH providers? ability to identify transfer-eligible patients and improve communication methods with PC to support the transfer of care. 000010057 542__ $$fIn copyright - single owner 000010057 650__ $$aHealth Services Accessibility$$019904 000010057 650__ $$aPrimary Health Care$$024590 000010057 650__ $$aMental Health$$022046 000010057 650__ $$aTransitional Care$$011384 000010057 650__ $$aPatient Transfer$$023675 000010057 650__ $$aVeterans$$027802 000010057 650__ $$aSuicide$$026555 000010057 691__ $$aSchool of Nursing$$041370 000010057 7001_ $$aGrant, Jennifer N.$$uOregon Health and Science University$$041354 000010057 7201_ $$aMcCracken, Kasey$$uOregon Health and Science University$$041354$$7Personal$$eChair 000010057 8564_ $$949f128ee-4ea8-4b47-b7b6-5e3fac2a3f26$$s634841$$uhttps://digitalcollections.ohsu.edu/record/10057/files/Grant.Jennifer.2023.pdf 000010057 905__ $$a/rest/prod/kh/04/dq/54/kh04dq54w 000010057 909CO $$ooai:digitalcollections.ohsu.edu:10057$$pstudent-work 000010057 980__ $$aTheses and Dissertations
