Despite efforts to create interoperability and communicate important information about transitions of care, the flow of information is inconsistent, not supported by electronic health record (EHR) functionality, dependent on workarounds within the primary care provider (PCP) practice, and has little feedback to promote quality improvement efforts. The question for this study is if a web-based transitions of care application would be more effective, efficient, and desirable than current methods in identifying and managing transitions of care, and which features would be desirable. The proposed application would aggregate information from the Health Information Exchange and present to the primary care provider in a way that would align with their workflow. 000002902 540__ $$fCC BY 000002902 542__ $$fIn copyright - single owner 000002902 650__ $$aPrimary Health Care$$024590 000002902 650__ $$aElectronic Health Records$$038928 000002902 650__ $$aHealth Transition$$031638 000002902 650__ $$aRecords$$025230 000002902 691__ $$aSchool of Medicine$$041369 000002902 692__ $$aDepartment of Medical Informatics and Clinical Epidemiology$$041422 000002902 7001_ $$aMcCarrick, Thomas$$uOregon Health and Science University$$041354 000002902 7201_ $$aDorr, David$$uOregon Health and Science University$$041354$$7Personal$$eAdvisor$$eMentor 000002902 8564_ $$9e416f3f1-b1e3-4fee-8d6f-0f01a073c264$$s341229$$uhttps://digitalcollections.ohsu.edu/record/2902/files/3672_etd.pdf$$ePublic$$2a4cbff5d9735d282e13e533a026adc5a$$31 000002902 905__ $$a/rest/prod/z3/16/q1/89/z316q189b 000002902 909CO $$ooai:digitalcollections.ohsu.edu:2902$$pstudent-work 000002902 980__ $$aBiomedical Informatics
