TY - GEN N2 - Congestive Heart Failure (CHF) is a serious and complex disorder affecting 6 million Americans. While there is no cure for CHF, quality and length of survival can be impacted by optimizing medical management. This medical management though often requires the coordination of numerous specialists in multiple fields including medical, social services, transportation, referral system and last but least the patient. The aim of this project is to identify three potential barriers to CHF care delivery that could be improved by using Agile and User Centered Design Integration (AUCDI) methodology.
