The scope of this project focuses on what lessons were learned from the Gresham location through interviews with staff, brokers, and leadership, and from the research turn that into a best practice guide for opening new locations such as the clinic in Happy Valley. These best practices also include strategies to convert the partnership's target demographic to Humana members and then align them with a Vancouver Clinic medical home. The recommendations in this report are made to influence three core goals: attract, grow, and retain patient membership at the partnered alliance clinic. Consideration was made to the short and long-term goals of the clinic, namely a transition to value-based care.
