The complete use of the bundle included the use of the problem list and medical history to document a hypertensive diagnosis, as well as adding a specific smartphrase to the discharge summary to aid with postpartum education and proper follow-up. This project was designed to implement organizational changes by increasing the utilization of the EMR to enhance organizational and practice efficiencies and improve the outcomes for people at risk for a pregnancy or postpartum hypertensive disorder. }, number = {ETD}, doi = {https://doi.org/10.6083/bpxhc42716}, recid = {42716}, address = {2024-03-05}, }
