It consisted of a cross-sectional retrospective review of data from a 2022 Uniform Data System diabetes report which revealed disparities in hemoglobin A1C (HbA1C) control between patients aged 18-29 and patients of all other age groups. The review elucidated clinic-specific factors contributing to these disparities and identified ways clinicians may optimize and improve HbA1C control for emerging adult patients with diabetes. 000042877 540__ $$fNo License 000042877 542__ $$fPublic domain 000042877 650__ $$aTransition to Adult Care$$039721 000042877 650__ $$aDiabetes Mellitus$$017671 000042877 650__ $$aBlood Glucose Self-Monitoring$$028204 000042877 650__ $$aGlycated Hemoglobin$$020045 000042877 650__ $$aYoung Adult$$038480 000042877 6531_ $$afederally qualified health centers 000042877 691__ $$aSchool of Nursing$$041370 000042877 7001_ $$aCullen, Olivia$$uOregon Health and Science University$$041354 000042877 7201_ $$aSoffer, Jonathan$$uOregon Health and Science University$$041354$$eChair$$7Personal 000042877 8564_ $$9bbe93414-ae0a-4898-bfad-cb9716820c82$$s539783$$uhttps://digitalcollections.ohsu.edu/record/42877/files/Cullen.Olivia.2024.pdf 000042877 909CO $$ooai:digitalcollections.ohsu.edu:42877$$pstudent-work 000042877 980__ $$aTheses and Dissertations 000042877 981__ $$aPublished$$b2024-04-23
