Patients with chronic disease if treated promptly and with evidence-based care can avoid many of these disabling complications prolong lives and reduce resource consumption. Chronic diseases require more proactive health care than our current health system allows. Decision support and clinical information systems are two of the six elements in the chronic care model and strategies such as guideline adherence feedback can help healthcare providers improve the quality of care that they provide to patients with chronic illnesses. This project aimed to find an acceptable method of presenting guideline adherence or profiling data to primary care providers in a chronic diabetes clinic. 000000445 542__ $$fIn copyright - single owner 000000445 650__ $$aDecision Support Systems, Clinical$$032052 000000445 650__ $$aQuality of Life$$025032 000000445 650__ $$aDiabetes Mellitus$$017671 000000445 650__ $$aHealth Information Systems$$040147 000000445 691__ $$aSchool of Medicine$$041369 000000445 692__ $$aDepartment of Medical Informatics and Clinical Epidemiology$$041422 000000445 7001_ $$aNorcom, Amy$$uOregon Health and Science University$$041354 000000445 7201_ $$aDorr, David$$uOregon Health and Science University$$041354$$7Personal$$eAdvisor 000000445 8564_ $$9300c3a77-2e22-4826-ac01-9fd86bde538a$$s510134$$uhttps://digitalcollections.ohsu.edu/record/445/files/446_etd.pdf$$ePublic$$299fca3fc7461a9df16af5e03f2abb1c3$$31 000000445 905__ $$a/rest/prod/w6/63/43/61/w6634361c 000000445 909CO $$ooai:digitalcollections.ohsu.edu:445$$pstudent-work 000000445 980__ $$aTheses and Dissertations
