This capstone examines why Medicaid and CCO funding mechanisms for temporary medical respite housing remain underutilized at hospital discharge for patients experiencing houselessness. Drawing on 21 stakeholder interviews across hospitals, coordinated care organizations, community providers, and Oregon legislators, the team identifies three root causes (a timing gap, an information gap, and a capacity gap) and proposes five strategic recommendations centered on formalizing and scaling OHSU Health Plan's existing same-day authorization model.
