Thoracic endovascular aortic repair (TEVAR) is the preferred repair method for blunt thoracic aortic injuries (BTAIs), but optimal timing remains unclear, especially in high-grade BTAI. Compared to patients without severe SOI, those with severe SOI had higher injury severity scores, lower admission systolic blood pressure, higher heart rate, and higher lactate (all p<0.01), while repair timing and BTAI grade distribution were similar. Early TEVAR in this cohort did not worsen outcomes, regardless of concomitant SOI, suggesting that delayed repair may be appropriate for selected patients.
