Dementia affects millions of older adults and is associated with high rates of preventable hospitalization, particularly among individuals residing in memory care and long-term care facilities. This paper reviews evidence-based strategies to reduce hospitalization risk through caregiver and staff education. Three major modifiable contributors to hospitalization are examined: falls, malnutrition and dehydration, and medication-related harm from polypharmacy. Effective interventions include supervised balance and strength training programs, individualized nutrition and hydration support, routine nutritional screening, pharmacist-led medication reviews, and deprescribing when appropriate. By empowering caregivers to recognize early warning signs and implement preventive measures, care facilities can improve safety, reduce acute care utilization, and enhance quality of life for adults living with dementia.
