000000478 520__ $$aThis pilot study evaluated the feasibility and acceptability of advanced practice nurse (APN)–led multidisciplinary group clinic visits for secondary prevention of coronary heart disease (CHD) in an outpatient cardiology setting. The intervention involved a team consisting of a nurse practitioner, dietician, social worker, and clinical pharmacist, and examined whether participation improved risk‑reduction behaviors and clinical risk‑factor outcomes in patients with established CHD. The Doctor of Nursing Practice (DNP) program strengthened the project leader’s skills in evidence‑based practice, team leadership, and organizational change, supporting the successful implementation of this model of care. Findings informed ongoing consideration of adopting the group‑visit program within the cardiology clinic and regional system.
