This study analyzed 21,302 patients undergoing screening or surveillance colonoscopy to identify predictors of serious complications and clinically significant neoplasia (>9 mm). Multivariate logistic regression assessed risk factors and estimated the number needed to endoscope (NNE) for each outcome. Serious complications occurred in 0.31% of patients, while 8.76% had large neoplasia. Anticoagulation therapy significantly increased complication risk without improving yield. Predictors of neoplasia included male sex, older age, ASA class ≥II, and prior positive screening. Aspirin/NSAID use appeared protective. Findings suggest screening benefits may diminish at older ages as complication risk rises, and anticoagulation markedly elevates risk without added benefit. 000000602 540__ $$fCC BY 000000602 542__ $$fIn copyright - single owner 000000602 650__ $$aIntraoperative Complications$$020972 000000602 650__ $$aColonoscopy$$016908 000000602 650__ $$aRectum$$025241 000000602 650__ $$aPostoperative Complications$$024462 000000602 650__ $$aColorectal Neoplasms$$028197 000000602 650__ $$aMale$$021762 000000602 650__ $$aRisk Factors$$025522 000000602 650__ $$aAnti-Inflammatory Agents, Non-Steroidal$$014842 000000602 650__ $$aAnticoagulants$$014868 000000602 650__ $$aColon$$016901 000000602 6531_ $$asurgery 000000602 6531_ $$acancer 000000602 691__ $$aSchool of Medicine$$041369 000000602 692__ $$aDepartment of Public Health and Preventive Medicine$$041444 000000602 7001_ $$aWilliams, J.
