The increase burden of PAD in this population has prompted providers to seek early identification and treatment. This project seeks to explore the usability of a simplified foot screening tool in a hemodialysis unit of a large urban teaching hospital. The tool focuses on the collection of physical assessment findings, followed by risk stratifying the findings into an action plan.
