Cancer survivors may also have unmet needs not previously known or addressed in their survivorship care. The goals of this program evaluation include: (1) identify unmet needs of Oregon cancer survivors, (2) explore patient-reported use of survivorship care plans in Oregon, and (3) assess patient perceived survivorship care plan efficacy in addressing unmet needs. 000008051 542__ $$fIn copyright - single owner 000008051 650__ $$aSurvivorship$$012208 000008051 650__ $$aCancer Survivors$$012117 000008051 650__ $$aSurveys and Questionnaires$$025037 000008051 650__ $$aPatient Care Planning$$023662 000008051 650__ $$aQuality of Health Care$$025031 000008051 651__ $$aOregon$$041094 000008051 691__ $$aSchool of Nursing$$041370 000008051 7001_ $$aCzorny, Alexis$$uOregon Health and Science University$$041354 000008051 8564_ $$990b15e8f-85b6-4c55-962c-554e107ed0bc$$s499887$$uhttps://digitalcollections.ohsu.edu/record/8051/files/Czorny.Alexis.2020.pdf 000008051 905__ $$a/rest/prod/br/86/b3/94/br86b3944 000008051 909CO $$ooai:digitalcollections.ohsu.edu:8051$$pstudent-work 000008051 980__ $$aTheses and Dissertations
