Yet evidence shows that people with SMI are less likely to receive standard care for medical conditions. Integrated care, or the process of addressing physical and mental health concerns together, has been shown to increase quality of care of patients, as well as reducing long-term health costs. As such, increasing integration has been identified as a goal for a community mental health center (CMHC) in a rural Oregon coastal county. This DNP project aimed to assess the current state of integration at this CMHC and to propose a workable program design and evaluation plan for increasing integration to best serve its highest risk members. 000009097 542__ $$fIn copyright - single owner 000009097 650__ $$aProgram Evaluation$$028379 000009097 650__ $$aCommunity Mental Health Centers$$016948 000009097 650__ $$aMental Health$$022046 000009097 650__ $$aDelivery of Health Care, Integrated$$031297 000009097 691__ $$aSchool of Nursing$$041370 000009097 7001_ $$aAldinger, Kelly$$uOregon Health and Science University$$041354 000009097 8564_ $$990bc03e7-64e7-4527-ab95-e8cbe89ae160$$s552394$$uhttps://digitalcollections.ohsu.edu/record/9097/files/Aldinger.Kelly.2021.pdf 000009097 905__ $$a/rest/prod/w3/76/37/65/w3763765f 000009097 909CO $$ooai:digitalcollections.ohsu.edu:9097$$pstudent-work 000009097 980__ $$aTheses and Dissertations
