Multnomah County Health Centers treat a diverse population of patients with type 2 diabetes, many of whom are not receiving regular diabetes care. The Health Resource Services Administration requested a thorough examination of patients with type 2 diabetes treated in Multnomah County Health Centers who had gaps between hemoglobin A1C rechecks greater than 1 year in duration.
