Previous work has shown frequent errors in medication lists include incomplete records, duplicated prescriptions, and failed discontinuation of medications. Since medication lists are inaccurate, physicians often record medication information in progress notes, which is difficult to automatically extract since notes are written as free-text narratives. In this study, we developed and validated a named entity recognition model to identify current medication and adherence from progress notes. Also, a prototype tool for medication reconciliation using the developed model was demonstrated. 000009225 540__ $$fCC BY 000009225 542__ $$fIn copyright - joint owners 000009225 650__ $$aMedication Reconciliation$$039494 000009225 650__ $$aData Collection$$017385 000009225 650__ $$aElectronic Health Records$$038928 000009225 650__ $$aData Accuracy$$011271 000009225 650__ $$aGlaucoma$$019541 000009225 650__ $$aNatural Language Processing$$022716 000009225 650__ $$aEye Diseases$$018815 000009225 650__ $$aOcular Hypertension$$023148 000009225 650__ $$aMedical Records$$021947 000009225 650__ $$aInformation Storage and Retrieval$$028998 000009225 6531_ $$aEHR 000009225 6531_ $$amedication lists 000009225 6531_ $$aclinical notes 000009225 692__ $$aDepartment of Medical Informatics and Clinical Epidemiology$$041422 000009225 7001_ $$aLin, Wei-Chun$$uOregon Health and Science University$$041354 000009225 7001_ $$aChen, Jimmy S.$$uOregon Health and Science University$$041354 000009225 7001_ $$aKaluzny, Joel$$uOregon Health and Science University$$041354 000009225 7001_ $$aChen, Aiyin$$uOregon Health and Science University$$041354 000009225 7001_ $$aChiang, Michael F.$$uOregon Health and Science University$$041354 000009225 7001_ $$aHribar, Michelle R.$$uOregon Health and Science University$$041354 000009225 711__ $$aResearch Week$$uOregon Health and Science University$$d2021 000009225 8564_ $$9f2f93eb5-e6f2-44d9-9d8f-4566042c9527$$s106429$$uhttps://digitalcollections.ohsu.edu/record/9225/files/Lin-Wei-Chun-OHSU-ResearchWeek-2021.pdf 000009225 905__ $$a/rest/prod/q5/24/jp/38/q524jp38n 000009225 980__ $$aResearch Week
