      abstract = {While not statistically significant, a differential impact  of SUMMIT on patient activation by age was observed. SUMMIT  patients in the 53-59 age range experienced the greatest  increase in PAM score over 6-months. These results suggest  there are differences in how SUMMIT affected patient  activation, reflecting differences in medical and social  conditions experienced during these life periods.},
