Implementing routine referral for outpatient prehabilitation in adults with end-stage heart failure under evaluation for heart transplant or ventricular assist device

In adult heart failure (HF) patients under evaluation for heart transplant or ventricular assist devices (VAD), preoperative participation in cardiac rehabilitation (prehabilitation), has the potential to improve functional capacity and reverse frailty. The aim of this quality improvement project was to develop a process for routine referral to prehabilitation in adult HF patients who are under evaluation for heart transplant or VAD.
