Abstract
In light of rising rates of youth death by suicide both nationally and in Oregon, this DNP project sought to administer the Ask-suicide Screening Questions (ASQ) tool to 75% of all eligible 12- to 18-year-old clinic visits at a newly established School-Based Health Center (SBHC). Data was collected for two different Plan-Do-Study-Act (PDSA) cycles between October and December 2023. Recommendations and possible future steps were presented to the clinic that included further increasing ASQ screening at the SBHC, expanding to other SBHCs, identifying community resources available to patients, and cementing clear management policies for follow up post risk identification.
