Abstract
This quality improvement initiative aimed to enhance the documentation of shared decision-making (SDM) in a faculty nurse-midwifery practice through the implementation of a standardized template. Over three Plan-Do-Study-Act (PDSA) cycles, midwives reported greater satisfaction (p = 0.035) and efficiency (p = 0.008) with the structured template compared to prior documentation methods, as well as improved ability to track SDM conversations over time (p = 0.003). While adoption varied, the findings highlight the potential for standardized SDM documentation to improve continuity of care without increasing workload, though better EHR integration, training, and workflow alignment may enhance usability and consistency.
