The inability to promptly receive outpatient psychiatric services post-discharge for patients recently hospitalized for mental health is associated with an elevated risk of suicide and readmission to acute settings. This project examines improving outpatient follow-up rates for patients recently discharged from psychiatric hospitalization at a CCBHC.
