Early organ donor card, developed by Vincent Glaudin of the Portland VA Medical Center; constructed in accordance with the Uniform Anatomical Gift Act of 1968 as part of a study on attitudes regarding organ donation. Signed by the donor, Loren Pankratz, this card is thought to be one of the earliest donor cards, if not the earliest, in existence.
