Abstract

This quality improvement project evaluated whether implementing a structured medication adherence screening tool could improve clinicians’ ability to identify medication nonadherence among veterans with serious and persistent mental illness. The project was conducted in an outpatient mental health setting and involved integrating the screening tool into routine clinical encounters. Outcomes focused on feasibility, clinician perceptions of usefulness, and whether the tool improved the identification of adherence barriers to support more individualized treatment planning.
