We confirmed that a 15 HU difference in medullary renal attenuation on contrast-enhanced CT is a highly specific indicator of renal obstruction, with larger attenuation differences associated with greater renal injury. These findings support the diminished nephrogram as an objective imaging marker of obstructive uropathy.
