This project was undertaken as a program analysis to evaluate a multidisciplinary diabetes management team in a free clinic serving low income, uninsured, type 2 diabetics. The purpose of this clinical inquiry project was to evaluate if the DMT was making a significant difference in chronic care management. To answer this, eight biophysical markers: glycosylated hemoglobin, systolic and diastolic blood pressure, weight, total cholesterol, triglycerides, low density lipoprotein and high density lipoprotein, were compared in 50 individuals meeting study criteria.
