Abstract

Age-Friendly Health Systems (AFHS) is a program designed by the Institute for Healthcare Improvement (IHI) in partnership with the John A. Hartford Foundation, the American Hospital Association, and the Catholic Health Association of America. AFHS is a framework based on incorporating evidence-based practices into care in a variety of settings including inpatient, outpatient, and post-acute long-term care (PALTC). Further research on incorporating depression, cognitive impairment, and delirium screening in the LTC setting will be important in ensuring evidence-based care and shaping future AFHS guidelines.
