Abstract

Background: Type 2 diabetes is a condition of particular concern, especially among groups with healthcare disparities. Multnomah County Health Centers treat a diverse population of patients with type 2 diabetes, many of whom are not receiving regular diabetes care. Purpose: The Health Resource Services Administration requested a thorough examination of patients with type 2 diabetes treated in Multnomah County Health Centers who had gaps between hemoglobin A1C rechecks greater than 1 year in duration. It was theorized that socioeconomic and disease-related factors were among the causes for these overdue A1C checks.
