Abstract

Inadequate transitional care and gaps in communication lead to fragmented patient care with increased frequency of errors and care coordination failures. Evidence supports use of standardized discharge documentation to reduce errors in handoff and increase provider satisfaction. The purpose of this quality improvement (QI) project was to standardize the transitional care of hematopoietic cell transplant (HCT) patients at discharge and improve provider handover.
